We were founded in 2008 by pre-eminent psychiatrists and psychologists and treat 6,000+ patients per year. With the critical need for mental health care and eating disorder treatment, especially during the Covid-19 pandemic, we’re looking for passionate and qualified professionals who will live out our mission to help save the lives of our patients.
What you’ll be doing:
This is by no means a “behind-the-scenes" role, as your work will help the company serve patients daily in a multitude of ways.
The Partnership Care Navigator serves as a strategic liaison and single point of contact between our organization, strategic partner providers, and the patients and families we serve. This role applies advanced knowledge, judgment, and independent decision-making—within established processes and in collaboration with the clinical care team—to foster strong relationships, improve patient outcomes, and enhance the overall care experience. The Care Navigator is accountable for developing processes that streamline access to care and resources while ensuring that partnership strategies align with organizational goals and regulatory standards. They are responsible for addressing inquiries, providing proactive and timely updates, and anticipating the needs of patients, families, and partner providers to ensure a cohesive and supportive care journey.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Strategic Relationship Management: Serve as the primary referral liaison for strategic partner providers, exercising discretion within provided guidelines in managing partnerships to ensure alignment with organizational objectives. Develop strategies to strengthen provider engagement and maintain high-value partnerships.
Patient & Family Advocacy: Guide patients and families through the admissions and care navigation process by applying independent judgment within defined protocols. Provide education on available resources, explain care coordination steps, and assist families in understanding next steps. Collaborate with the care team to ensure that patients receive appropriate clinical guidance, while making non-clinical decisions to remove barriers to care.
Care Coordination & Program Oversight: Coordinate efforts between internal teams and external partners to support continuity of care. Make decisions on referrals, scheduling, and resource allocation within established workflows and in collaboration with the clinical, partnership, and admissions teams. Monitor referral patterns, evaluate program effectiveness, and proactively resolve barriers that impact patient access and outcomes.
Data Analysis & Reporting: Analyze data and trends related to referrals, patient satisfaction, and partnership performance. Prepare reports and recommendations for leadership to inform strategic decision-making and continuous improvement.
Issue Resolution & Risk Mitigation: Use independent judgment to identify, address, and resolve complex concerns involving patients, families, and partner providers. Escalate high-risk or sensitive issues to leadership or clinical staff as appropriate, with recommended solutions

Compensation range for the Partnership Care Navigator role is $50,000 - $70,000. Ranges are based upon market expectations and are dependent upon candidate experience.
